ADMISSION OF STUDENTS AND APPLICATION REQUIREMENTS


Applications are now being accepted

for the 2010 - 2011 school year for

Pre-K age 3 to

Grade 7

Age requirements before December 31st:


  • Pre-Kindergarten- 3 and 4 year olds

  • Kindergarten - 5 years old

  • Grade 1 - 6 years old

Readiness tests are administered to all applicants for Kindergarten and Grade 1.  Applicants for Grades 2 and above are also tested when needed.

NEW YORK STATE HEALTH REQUIREMENTS FOR NEW STUDENTS


  • Physical Examination

  • School Health New Admission Examinations

  • Immunization requirements per grade level

Students cannot be admitted to class without required immunizations.


 

APPLICATION REQUIREMENTS

Photocopies of the following must be submitted, at the time of registration, for all students:


  • Birth Certificate

  • Sacramental Certificate (as appropriate):  Baptism. Reconciliation, First Communion and Confirmation.

  • Immunization

  • Proof of Residence (only one of the following):  Apartment Lease, Rent Receipt,Telephone Bill or Con Edison Bill.

  • Parents registering students in Grades 2 and above must also furnish the following:  Student's Report Card, NYC Reading & Math test results, ITBS scores ( Catholic Schools), New York State test results.

  • $25.00 non-refundable application fee.



REMEMBER that students of all faiths are welcome to register at Sts. Anthony and Frances School.
